This facility participates with Medicare, Michigan Medicaid, and Blue Cross/Blue Shield. In the event of any other 3rd party insurance we will assist you in completing the necessary forms for reimbursement to you.

Private Pay

The patient or responsible party will receive a monthly statement. This amount is payable during the first week of the month. Private Pays are charged the daily rate for day of admission and day of discharge. In the event of discharge, you will receive a full refund for any unused days.

Medicare

If a patient meets the SKILLED GUIDELINES to qualify for Medicare, they are entitled up to 100 covered days per calendar year. The first 20 days are covered 100% by Medicare, day 21 thru 100; the patient must pay a Medicare Co-Insurance of $101.50 per day. If funds are not available to pay this amount privately, you will need to make application through Family Independence Agency for Medicaid coverage to help cover this Co-Insurance amount.

Medicaid

If funds are not available to pay the costs of Nursing home care, you will need to make application at the local Family Independence Agency located at 1911 Sand Beach Rd., Bad Axe, MI. Phone number (989) 269-9201. At that time you will be assigned a caseworker that will review your case and determine eligibility. The worker at FIA, based on the information provided by you, will determine the amount you will have to pay the Nursing Home each month, and the effective date that payment will begin.

Our Facility will send out monthly Patient Pay statements for Medicaid recipients. Payment is due the first week of the month, or as soon as Social Security checks arrive. The Family Independence Agency determines any change in the monthly Patient Pay amount.
